Yes movie pass is accepted and only for 2d movies, even some of the movies are restricted, please check the movie pass app for movies that are accepted.
The card is accepted but not for all shows.
Movie pass is accepted at all regals. As long as you can see the movie you want in the app you can get a ticket for it at a regal theatre. There shouldn't be any other fees once you obtain your ticket through the app
Sorry i have never used it. But they do accept gift passes that i used and they have accepted. They do charge extra for 3D and RPX so you might want to call them and check.
