What exactly is your employer looking for? Remember your personal medical results are protected under HIPAA, so the doctors office is restricted from giving out that information legally.

Make another appt and when they ask what for say needing note to return to work. Then when you have someone face too face explain the situation and see what they will/can do for you.

Look them up on Google and call the phone number shown, then when it prompts you, push the button for customer service. They will connect you with whom you need to talk to so you can get what you want.
