I'm not sure if it costs more without medical records. I think it does. When I had my appointment, I was registered the same day. You just have to wait for your card in the mail. I think if you want to get it without medical records, saying that you have anxiety would be enough but I'm not sure. The doctor that I saw there was awesome!
