Board of Health Law requires that all minors (age 14-17) must come accompanied by their parent or legal guardian. We need an ID from the adult, an ID from the minor, and the minor's birth certificate. The name of the parent on the birth certificate must match the name on their ID. If the adult is a legal guardian, guardianship papers are required to prove legal guardianship. *This is a Board of Health Law and by no means are we able to bend or break this law. Adults, age 18 and up must have their ID to receive services, that's it!
