Im not sure, but I think you need to call whatever service you used to schedule your appointment and figure out how to get a replacement.

Have you had your 2nd shot yet.. if not then maybe they can replace it when you go back.. If you've already have your 2nd shot then probably calling the Tinley Park Convention Center to see if someone can help you by possibly mailing you a new one would be your best option..
