Last I checked u will needproof of address (utility bill or bank statement) social security card, and birth certificate
Your current valid license and a bill in your name at your current address. A utility bill will do just fine. Also, they were kind enough (and sensible enough) to use the utility bill I pulled up on my phone seeing as how I pay all of my bills online and don't receive paper ones.
