Only for alcohol purchases, or on certain nights there may be a cover for an event, but generally no, not to get in.
Not in my experience, but likely needed to buy alcohol.

There is never a cover. 21 years of age and older after 10pm every night.
I think it depends on the time. We got there around,midnight and GOT ID'd to go in
