unfortunately we do not provide nostril piercings for 15 year olds. The person getting a nostril piercing has to be 16 or 17 with parental consent. Both last names have to match or addresses have to match.

Yes, bring birth certificate and any other ID she might have, your name needs to be on her birth certificate
