Social security card, birth certificate or passport, two proofs of address... original documents only and no birth certificates that say birth registration
1)Your birth certificate or passport, 2) your social security card and, 3) 2 pieces of mail with your address (cell phone bill, utility bill, bank statement, mortgage statement, leasing agreement, credit card bill, voter registration card, title or vehicle registration card). No copies will be accepted.
