Yes you have to have proof from your Dr with your qualifying condition.

Not sure on that one I know he did mention having patients that didn't have their diagnosis from a doctor handy but he never said anything about if he himself diagnosed or not. My girlfriend provided proof. her process was easy and quick I'm sure it would be more of an ordeal if she hadn't. You could always call and double check before making an appointment
