You first must find a qualified doctor who is certified to recommend marijuana. Then your dr enters you into the registry and you can fill out your application on your phone if you have a smart phone - ask your dr to place your app so you can retrieve it online. Or after that visit go to the Florida Department of Health to pull up your app. You will need to photograph your drivers license & either take photo of yourself with dimensions 2" × 2" - I suggest getting a passport photo at Walgreens or anywhere inexpensive. Then take photo of the photo & will be correct size. Best of luck!
