There are requirements that include a consent form being notarized yes:) I also strongly suggest making an appointment when ready:) here’s the run down I generally send people regarding minors: NOTICE: PARENT/GUARDIAN PLEASE READ THOROUGHLY:) As required by the Florida department of health,and in order to comply with state law, the consent form link I provide needs to be printed out, and notarized, by the parent/guardian prior to coming. I then need photo ID for the minor and the adult who gets the paperwork done. If the minors ID isn't a state/govt one (such as a yearbook or school ID), and/or if last names differ, then I need a birth certificate as well. If the guardian is not the biological parent and/or does not appear on the birth certificate, then you also need the documents showing legal guardianship. There WILL NOT be exceptions made and all that is mentioned will be required prior to being pierced. (It won’t allow me to attach the link to the notary form)
