As far as I know. You'll need the child's certificate of birth and have selected the Godparents. Godparents need to have their confirmation if single and if married it has to be married by church. You go to the office to sign up for it and select the date.
I would have to go talk to the Rector. It seems to me that parents and godparents should take some classes.
