Where do I mail my rebate form and receipt to? There is no address on the form. I live our of town and we purchased the items at Menards on Maize Road, Wichita.

Did you pick up a rebate form from the front of the main desk.ny the exit? The form has all blue writing on it and it has the address to send it to.
