Yes we accept Staywell, they do have some requirements, they want your primary care physician to send you to an optometrist first, then you would be able to see the ophthalmologist with an authorization that the PCP has to request from Staywell. 7.16.2020 The newest requirement from Staywell is that you must be referred by your PCP
Make sure that you make two appointments a year or you will get one after the other to fill in the year charges.. So make them six months apart.
