Your best route is to go to their website. There are very few situations where they will issue someone a replacement social security number. If you go to the website you should be able to find what you need, probably under the topic of identity theft. From there you'll find out what proof and documents you will need to apply for a replacement number. If you have just lost your card, you can request a replacement one free of charge.
