If you have medicaid it doesn't transfer state to state. You would need to reapply. If it's county to county then once you update your address you would just need to find a new doctor.
You come into the medicaid office here at DSS and depending on a few questions asked we will direct you to one of tow places within the building. You might be able to stsrt process online or over phone
