My daughter was 14 years old when her last passport was issued and now she is 19, What is the correct form to fill out for her to renew her passport?

Form DS 11 will need appointment at Post offices that process passports, like Turkey Lake Office not all do. Offices are booked solid, however she can walk in Orange County courthouse no appt necessary. Will need original birth certificate, DS 11 filled out in black ink, old passport, driver's license. Also Check/Money Order for passport fee, Court House can't provide that.

U can go to the post office and get a new passport. The same procedure u used when she was 14 still applies now. Even better she/he can go by herself.
