For our Value Plan only, the following applies: You pay $15 for each Medicare covered exam (diagnosis and treatment for diseases and conditions of the eye); you pay $30 for up to one routine eye exam every year; you pay $0 for one pair of eyeglasses or contacts after cataract surgery; you can be reimbursed up to $100 for contact lenses and eyeglasses per calendar year.

For 2019 the value plan covers a routine eye exam once a year with a $30 copay. They cover a $15 "Medicare covered" exam under a $15 copay. For vision hardware (lenses, frames, and contacts) we offer a reimbursement of $100
