You have to fill out a Request for Replacement Placard form, can be found on the mass.gov site or you can visit your local RMV Branch to have one given to you. It then needs to be either mailed back to haymarket or faxed to the number on the form
You have to go thru the haymarket location to get assistance they are the only location that deals with hp placards
