Medmar Dr.Israel Stein 59 Coddington St Quincy Ma 02169 Dr. Stein will do your assessment and the office staff handled all the paperwork. Office fee for this is $150 and the State charges $50 fee for card. Their prices were comparable to every other place I've heard of. Very nice, helpful staff. Doesn't overbook. Card arrived in the mail about 10 days later. I would call and get current information from them. I know they were willing to work with people who needed to split the payments.
