Call or visit them and help them look it up and you can have it mailed to you or pick it up
Just call them. Give them your info, and then you can go and pick it up.

I went there yesterday for the exact thing. They took a copy of my driver's license and social security number logged into some system somewhere in printed my form right out for me but I'm talking 1099 g but I'm sure they could help you.
