Florida law states that tattoos can only be done on 16 and 17-year-old minors. Parents have to be present, or legal guardians, and both must have identification. If last names don’t match, there is a form that must be filled out and signed, along with being notarized. In addition there is other paperwork that needs to be filled out. 15 and under cannot be tattooed per Florida law.
