Not all of the offices process those so you will have to check with an office or call the main 800 number for Auto Club which you will find online and figure out which one to go to. Now they may have changed because when I got my temporary placard I had to go to only one office but it seems that they changed something maybe they expanded that service and I was able to go to another office closer to me when I got my permanent placard and those are free the temporary Ones cost some money not much but they do cost. Remember to do some of your business on the phone so you're not driving all over with these high gas prices just hoping that they will handle what you need that'll save you some money right there

AAA process and issues both temporary and permanent placards. If the Dr. does not have the paperwork to fill out, it can be downloaded from the DMV website. Fill out her information and then take ro the Dr. to fill out the remaining information and his or hers signature. Then take that to AAA. Thats it.

From what I understand you need to apply for placards at DMV. You also would need her doctor's verification. You can find the application online and submit it with all supporting documents via mail as well.
