You will need a photo ID, two items that prove residency (utility bill, mortgage... etc), birth certificate and social security card. If your name has changed between your birthday certificate and social security card then you need your marriage license or official document showing the name change.
I had to have a utility bill showing my address. My old license and birth cert. I believw itvwas 2 piecws of mail showing address 1 being utility.
