I recieved forklift operator certificaton through goodwill industries in 2011. I need a replacement card mailed to my Clearwater address to verify my certification for potential employers. Who might i contact to accomplish that

Goodwill forklift certification is only good for 3yrs and it is Only valid for a Goodwill Employee
