What does it take to adopt a puppies or dog

I'm adopting a kitten. I paid $15 dls to put it on hold because it was skinny and was not ready to go home. I'm paying $60 dls when I pick it up. There are some questions to answer, if you have a place for it, where have you taken your animals to the vet, how many pets have you had and how long they last with you. What are you doing with it if you move and they don't accept pets at your new place. They need your drivers license number and will ask you how many members at your house. They also want to know if the pet is for you or a gift. I hope this gives you an idea.
