I would say business casual or upscale leisure based upon what I saw there.

I think business casual is the norm for the dining room. Ive seen very casual attire on the upper deck at happy hour - more casual than appropriate IMO. Jeans (sans rips) and a nice shirt are fine.
Depends on the time. We were there for lunch and came in wearing shorts and tshirts.
Casual. We walked in with jeans and shorts...no issue.
