State law in Wisconsin is 16 with parent or legal guardian consent. Ages 16-17 must have a parent present with their ID and valid identification and a birth certificate for the minor being pierced. At 18 you can sign for yourself.

I'm not positive, but I'm almost sure 15 with parent consent. I believe 17 without
