I went today and just used my phone without a problem
You can show your phone or bring a printed paper. Either one works. Just make sure the screen brightness is turned up, and have your barcode pulled up to show at the entrance.

I've always uses the barcode on my phone and never printed off. It hasn't been a problem.
Yes and no
They only scan the bar code. However, they may want to see it attached to a ticket. Best to ask the next time that you visit.
