You need to take your medical records stating that you have a chronic illness. Such as RA, FIBROMYALGIA, ETC. You have to make an appointment, usually takes 20-30 minutes, depending on how busy they are. You will meet with the Dr who will go over your medical records, ask you questions. Then you pay the fee. I think it was 199.00. Then they will give you a certificate to sign up online for the actual card. Once you pay the 50.00 for it you will receive in the mail usually 5-7 business days.
