Not that unusual for a night club with a premium specialized menu. They have several different hamburgers but they are served to the customer top bun to the side with the veggies for the customer to stack to his taste. They're goal is to serve food that people will remember for it's quality and abundance. The goal is to avoid request for more or extra items. They want you to enjoy the experience of having more than enough of everything that the meal included and to reduce and simplify order request from customer to server to cook. The kitchen staff are trained to prepare meals as written on the menu so that the customer knows they're getting the same premium specialized meal that they were expecting. The more respected and admired the restaurant (expensive-exclusive) the more likely you are to incur a service charge for any additions or substitutions. Subtracting from a specialized item creates quality assurance problems equivalent to a substitution.
