Our Mesa Location anticipates being open on Saturdays in the future, due to being a newly opened office our first operating Saturday will be August 25th, 2018 from 9-2 PM. Hello Johnnie, For a New card 150 to the state or 75 if you participate in the Nutrition assistant program through the state. If you are a new patient with Dr. Reeferalz and have a snap card with your name on it or the award letter stating that you were granted nutritional assistance the state fee would be $75 on a mastercard or visa, and the office fee would be $200 cash ( this includes the office fee and exam). If you have medical records within the last 12 months that our doctor could review and determine a qualifying diagnosis the office visit cost $150 cash. Depending on the exam, your total would be no more than $275. It is a two part payment. I hope this helps. Have a great day!
