THE PLASTIC CARD IS NOT THE LEGAL PORTION THE CARD IS MORE FOR CONVENIENCE ONCE YOU ARE ESTABLISHED WITH A DISPENSARY THEY MAY LET YOU ONLY USE THE CARD HOWEVER ANY TIME YOU GO SOMEPLACE NEW OR OUT OF TOWN YOU WILL HAVE TO HAVE YOUR ORIGINAL DOCUMENTATION. NEW PATIENTS ARE $80 CARD IS AN ADDITIONAL $20 $100 TOTAL FOR OUR OFFICE
