For Covid-19 vaccine for people with medical conditions it is asked to bring to your appointment "The COVID-19 Authorization Code from your state/local government (if applicable)". What is it? When is it applicable? How to get it?

The governor has signed an edict that you don't need to bring anything to your appointment. He is, however, asking that we abide by the honor system, which is to be honest about whether we have medical conditions when we arrive at the appointment. People who do have them should get the vaccine first.
