My mother has been to SH&R twice. After the 1st visit she came home with wounds on both feet. No one at SH&R notified of the injuries. Today my Mother was supposed to go to SH&R for a 5 day stay. I followed her to fill out paperwork and go over her needs. We arrived at SH&R around noon. I left to go get lunch at 2. Before 3 Mom's Hospice nurse called to tell she was on her way to SH&R because Mom had fallen. I was surprised that SH&R hadn't call me. However, SH&R did call me later to tell that they they were sending her home. When Mom arrived home she had another wound on her wrist. It was bandaged with butterfly bandages. No one at SH&R called to tell of the second wound. DO NOT SEND your loved ones to SH&R.

I wish I could give this facility a negative rating.My Father was there several years ago and they nearly killed him through neglect.If it hadn't been for one nurse on duty who called me and told me that he needed to go to the hospital he would have died in his bed at this wretched place I should have sued but decided against putting my Mom through that.If you need to find a rehab for your loved one please stay away from this one!

This place is awful they do not care about their patients. My grandfather was there and they left him in his bed did not take care of him only gave him a tiny amount of something to drink they claimed that his surgeon ordered that which was wrong. He wound up in the hospital his kidneys starting to fail, the nurse at the hospital said if he would have stayed two more days he probably would have died. This place is a nightmare. Please do not put your loved ones in this place.

There is access to a supply of medication for first dosing at the center that is available while new medication orders are communicated with the off-site pharmacy. Recurring prescriptions are filled by the off-site pharmacy for residents. Consultant pharmacists review each resident's medication list on admission and monthly thereafter to monitor each resident’s individual medication regimen and make recommendations to the physician.
