When a patient who is designated as a fall risk is transferred from a hospital to a nursing home who is responsible to make sure the nursing home staff knows the patient needs assistance from the bed to the bathroom, getting dressed, etc?

How it works goes like this; the hospital gives us the packet of ALL info on the resident. This includes fall risks, continence, assist and such. Once the resident is in our care, our therapy staff evaluates the resident and informs us of the type of assist needed further. So to answer your question it would initially be the hospital but then we evaluate them also.
