My Dad is currently a patient of this facility and on Aug 22 I contacted the phone number given to request help for my dad who desperately need a walker or cane to prevent him falling due to weakness from his illness. First there is no way to talk to the facility directly, you reach a customer service rep that documents your issue and only able to email the physicians assistant (they handle most issues before the Dr is even considered) , 3 days later I still hadnt received a call or update as to the status. so i called again to be advised the PA had gotten the message but there were no notes as to whether the devices were ordered or not. I was also advised the ENTIRE office, and most other OAK street health offices were at a conference and ALL of them were closed! I would likely not get a response till Aug 29th Monday. I have still not received a call from them. These policies and procedures are blatantly neglectful, and lack personal care or concern for patients health and well-being to say the least, needless to say i am seeking alternate care management for my Dad and if you love yourself or your family member I suggest you do so as well

I have always had pleasant experiences at Oak Street. I’ve never been in a situation where the staff didn’t care they were always very caring very personable and my experiences have always been pleasant. I would highly recommend that others go to their facilities to get the care.

Print attention, thorough care and efficient doctor and nurse’s care are attributes. Also efficient follow-up.
