Arguably the worst office I have ever been to. 3 workers were telling me totally different things, one lady was extremely rude. Very disappointing

Rude, arrogant, & refuses to explain a problem when asked. Staff should be retrained or fired. The fact that you hire such disrespectful individuals to work with the disabled is beyond me.

You have to wear a face mask, if not they will provide you one. The security guard was really nice and friendly, very helpful, the person who helped me with my paper work was also very nice, and answer all my questions, he worked fast. I was there at 930 in line and l was out at 950.

They lock the doors right at 4pm. I was able to enter around 3:50pm and they took care of what I needed.
If you are a US citizen, whether naturalized or by birth, you need valid ID. Valid ID can be a current US passport, state ID, or state drivers license. If you are a naturalized citizen and you have not been in previously to update your record, then you need your Certificate of Naturalization as well. For a name change, whether legal or due to marriage you need the documents for that as well. The only time you need a birth certificate is if you’ve never had a social security number assigned.

You give this form to your payer it says so right on the form. Pretty sure it has nothing to do with SSA. "Give this form to your paver. Do not send it to the IRS."
