Proof of residency i.e light bill, gas bill, lease, voters registration card or letter from irs. Also a Social security card and a photo id i.e school id, state id, license, gun card or e registered id with picture. Hopes this helps

Yes, but you can do it after you arrive and, at least in my case, they let me in as soon as I showed I made the appointment (even though it wasn't scheduled for another 1.5 hours)

I don't believe so. They definitely asked me for it. If you go to the DMV, don't wait in line. The employees outside are there to answer questions so you don't have to wait.
They have a person outside with a card with a QR code on it as well as a bulletin board with the code on it to set up an appointment. Do know you won't be able to schedule a driving test there for weeks possibly. You can go on 99th and King Dr. They still accepting walk ins. I hope this helps
