Dress respectfully and with modesty, as God resides in the Church. There's no really enforced code, but one is expected to dress as nicely as they are able to present themselves to the Lord.
All you need to do is walk through those doors. God is waiting for you to come home. Also, when you are ready, go to confession and tell the priest you have been away for a while. He will guide you back to full communion. I will be praying for you.
