My first visit to Dr. Barry Klyde was a comforting experience. I was very anxious about the recent diagnosis. My conversation with Dr, Klyde was reassuring, the treatment prescribed has been successfully.

Dr. Klyde has been my primary physician for more than 25 years. He's kind, considerate and a great listener. He has over the years referred me to other specialists when needed, all of whom have been stellar as well. I cannot recommend Dr. Klyde any more highly.

First, we will ask that you fill out a health history and registration form. We ask patients to arrive 15 minutes prior to their appointment in order to fill out this paperwork. The first intake visit is usually one hour, and is mostly one-on-one with Dr. Klyde. There is no set duration as each visit is tailored to the patient. We want to take the time to ensure that we evaluate a patient's situation carefully and provide the best care.
You can drop off any forms at our front desk, submit them by email, or fax them to us. It can take up to five days to complete these, depending on the type of form. There is some paperwork that could require you to set up an appointment with your physician to complete.
To receive a copy of your test results and medical records, please call the office. You will need to sign a consent form to release this information if your records are being sent to another office or hospital.
