My mother went into Oak Trace on Hospice care. We were private paying customers, at $423 per day. While I believe the facility is likely great for rehab or long-term stays- I would not select this facility for a patient in hospice. I do not feel the communication by their staff is adequate to actively care for their patients during their final weeks. The pros: The facility is spotless, free of odors, and the cleaning crews do a great job. The residents have exceptional meals and the food services workers are kind and polite. Activities are plentiful & they do try to engage their residents. Areas of Improvement: The arrival process was not good. Nobody met my mother at arrival. We were told to head up to her room. Nurses, CNA's and general staff had no idea she was on hospice or what her situation was. After several phone calls, and two hours later, we were finally met with someone from admissions. Staff was not aware my mother was on hospice for DAYS- even after we asked to ensure that this was communicated. Notes in the patient portal were not always accurate Doctors orders, such as wound care, were not always followed. My mother entered Oak Trace able to communicate, and with good cognitive function. Several times she asked for help to phone her family- the staff refused. It was not until we contacted administration were signs put up to assist her with contacting us. During her final days, she was actively dying. Staff would enter the room trying to feed her when she was unconscious. We would ask them to stop, as she was dying. CNA staff were not coming to turn her, provide mouth care, or check in on us. If we were present, they would not see what we needed. The facility also tried to administer a COVID booster the day she died. There seems to be a lack of awareness and empathy for individual situations.

My husband needed a sub-acute rehabilitation facility for inpatient physical and occupational therapy that also serviced dialysis patients. We decided on Oak Trace in Downers Grove and he spent nearly three months there. Thanks to the skilled nurses and therapists, his recovery has been remarkable. Everyone from the food service staff to the aide (John) who provided daily personal assistance was super friendly and attentive. We would recommend Oak Trace to anyone needing specialized inpatient therapy.

If you're a senior who is still living independently, consider the Independent Living options at Oak Trace. We have a vibrant community of seniors engaged in doing all there is to make life fun, active, and satisfying while giving up many of the everyday cares and chores that take up so much of the day maintaining a place for yourself. We have great food, with a diverse menu. Our support staff is terrific and helpful. We have more to choose from in social activities and well run programs than can ever be done -- if your choose to join in, just watch, or enjoy the solitude of our campus.

What level of care do you offer and do you take Medicaid patients? I’m thinking of moving my father from Prairie Manor Nursing Home in Chicago Heights closer as I live in Downers Grove.
