I think we paid about $37 a month before, but now we use a program through our workplace insurance where we pay $25 a month and can use AF or some other gyms around the area, so definitely check into that. They do not have Jacuzzi, steam room, or sauna. Most have a tanning bed and the Merrillville location had a hydro massage bed last I knew.
I think you have to go in when the employees are there only otherwise you won't be able to get in since you need a key card to open the door

Yes, I still have guess passes and check with the office. I think you can sign an insurance waiver and a 10 dollar fee.
Last I checked, Mondays at 6. But there is no instructor anymore. It's a video on the screen in the workout room.
