Hi! You would have submit photos on their website to be considered. And an agent should be in touch with you within 6 weeks. If you don't hear back submit again within 6 months.
Not up front. If they sign you and get you work, they take a commission. That's standard practice.

Within 6 weeks they will contact you only IF they'd like you to audition. On their website they outline how frequently to submit your materials. Best to play the field and reach out to multiple agencies but there are more effective ways to get an agent through your own network. Lose the anxiety! There's enough rejection in this industry to deal with those emotions every day.
Most of the agencies in Chicago get casting notices for movies. Lily's has a pretty good children's division.
