Yes, dogs are allowed if they are on a leash. Bring some bags to keep the trail clean, and have fun. It's a great trail!
There is a fire pit and pavilion at Kiwanis Park, which is right next to the trail, just off the parking lot.
99.9% as of end of January. The fall it self is frozen, top to bottom, but at the bottom of it there is a small stream of water an inch or so wide that is trickling down. Bring ice cleats to get there. Going down to the falls once you get there is the biggest challenge, coming back up from the falls to the trail was fairly easy. This was our experience on Jan 30th

Yes, you can go a couple miles up and find good camping spots. It is relatively steep, though, so with a full pack on you should be in good shape and/or take it slow.
