I visit here every 6 months they have removed a few skin spots they were pre cancer glad they are gone. Plan on spending wait time but when it is your turn they spend all the time you need. Worth the wait

Doc Perez, PA's and Staff are very nice, but, Doc, team and staff don't return calls. PA's won't listen to client for added requests. 12 appointments canceled this year , never has office rescheduled me. Doc/surgical team doesn't care because I am only a follow up. I thought JAX had a better cut of businesses.

Tried to call to schedule an appointment. When I called, I got the automated system telling me that I was twelfth in line. Waited for about 30 minutes to get to my turn and you guessed it....they hung up on me. So, I called back. This time I was thirteenth in line. Again waited for around thirty minutes, finally got to my place in line and they hung up on me. I will find another provider. If they can't get answering the phone right, I don't think I want them being responsible for my health.

I’ve been coming here for YEARS and I’ve never had a bad experience everyone is so helpful and welcoming I always recommend them to all my friends and family! These past couple months though one of the PA’s Carmen Somarriba WOW WOW WOW amazing! I always feel heard at my appointments and she goes above and beyond to figure out what will be the best option. The best dermatologist I’ve ever had. 10/10

Go for it! They prescribed phototherapy because I had severe symptoms (itching, redness, and cracks). After the phototherapy my symptoms improved significantly. No longer does it impact the quality of my life, and my overall health has improved, too.
Mine lasted for 4 hours, but the length of your Mohs surgery varies depending on how many layers of tissues your doctor has to take and when all of the cancerous cells are removed. The length of the surgery can range anywhere from 4 to 6 hours. But don't worry, they have well-thought-out policies and their appointments start early in the morning.
Because the doctor is money hungry
Your medical records are available at both clinics and yes you can switch back and forth between clinics as often as you like.
Yes, you can. You only need to tell your health provider at the time of your visit that you would like your orders to go to a particular laboratory. Note that you will get bills from the laboratory. Whenever I have questions regarding my bills I contact the lab for clarifications.
Yes, Molina Healthcare and Sunshine State Health Plan are both included in the list of their health insurance carriers. In fact, their providers are contracted with most major health insurance carriers in the region. You can find the full list on their website under the tab PATIENTS --> INSURANCE & PAYMENT. Here is the link: https://www.advantagederm.com/insurance-payment-jacksonville-fl/
I think if it doesn't clear up on its own you should definitely see a doctor. I have used Advantage Dermatology's services for my daughter and they have been really helpful.
You will be charged a no-show fee if you don't cancel your scheduled appointments 48 hours before the appointment. Fees differ based on the appointment type: $50-Clinical $50-Cosmetic $100-Surgical
Make sure you bring the following documents with you on every visit: 1. Driver's License or Identification Card 2. Primary Insurance Card(s) 3. Secondary Insurance Card(s) 4. Form of Payment (Cash, Check, Visa, Mastercard, American Express, Discover) 5. List of current medications (dose, route, frequency)
Im sure they could assist you. Did you have treatment there? If not let them know. They have shot you get so many times a year or something.
Yes, you may get a paper copy or summary of your health information. This is usually processed within 30 days upon your request and charged at a reasonable, cost-based fee.
CareCredit helps you pay for out-of-pocket healthcare expenses for you and your family.
Yes. CoolSculpting is a safe, effective non-invasive fat eliminating procedure. It uses Cryolipolysis® or cooling technology to target and kill fat cells. It does not require surgery and there are no downtimes so you could quickly go back to your daily activities safely. Perfect for any busy mom. Call the office for a free consultation.
I saw effects within 48 hours and lasted about 3.5 months. Some last longer.
Most doctor's offices do. The PDF form they have on their site (https://www.advantagederm.com/wp-content/uploads/2016/11/Medical-Records-Request-Form-From-Our-Facility.pdf) lists the fees there. Looks like general medical records requests are $1 per page for the first 25 pages and then $0.25 per page for each page in excess of 25 pages.
Any bloodwork and/or biopsies done in the office are sent to a laboratory, which will then bill your insurance for their portion of the work. You should deal with the laboratory directly if you have questions about that bill.
Sorry, not sure. They do accept most insurance companies including Medicare. Better off calling to verify.
You have to give them 48 hours (2 business days) notice that you won't be able to make your appointment, otherwise, they charge you a fee. I think the fee is $50 for regular appointment and $100 for a surgical appointment (procedure, etc.)
I got KYBELLA treatment here. KYBELLA and cool Sculpting are the only non-surgical treatments for a double chin (alternatives were liposuction, neck lifts, and laser treatments - no thank you).
I did my first fillers with Dr. Perez and they lasted about 8 months for me. Not sure if there is something that lasts less than that.
Yes, I highly recommend Advantage Dermatology, PA for laser hair removal -- they have the most effective hair removal and at the lowest price! What's also great is they have the latest, most up to date equipment, and having it done at a doctor's office just makes me feel so much safer.
Even though the office is 5 minutes from downtown center of Jacksonville, I've never had any issues with parking - they have parking across the street in the Center Bank parking lot and on site all around the building, more than 65 parking spaces
I take my 5-year-old to them and they have been excellent at addressing his skin issues.
This is a very well-run office; I've never had to wait for longer than 5 minutes. Usually, the nurse takes you back as soon as you sign-in.
Dr. Perez recommended that I get BOTOX 6 months before my wedding, in case touchups are necessary. The procedure itself is short but you need to allow time for redness and swelling to reduce.
Yes
It usually takes 45 days to two months to get things settled with the insurance company. But it all depends on your individual plan. I would contact them directly for more specific information.
All types of rashes should be taken seriously. A year ago my husband and I contacted Advantage Dermatology with a similar issue and they helped our 4-year-old baby. The service was prompt and of the highest quality. They are really caring and detail-oriented.
Yes. My family has been using their services for more than two years, and I always make sure I follow their policies. Appointments should be canceled 48 hours before the scheduled appointment.
Absolutely yes! He did my sister's surgery and we are more than pleased with the result. At the end of the procedure there was a tiny scar left, but it faded away with time. He has a great bedside manner, he is highly professional and caring. Look no further. I'm pretty sure your expectations will be met.
