This dcf office is the worse. It needs new employees the lady at the front with the rotten teeth that works there, I gave her my paperwork to fax 2 times and she didnt fax my paperwork. And she is just so rude. I have never been to a dcf office and treated like this nor have i ever had this happen.

If an individual wants to apply using a paper application, the Family-Related Medical Assistance Application is for individuals who desire to apply for medical assistance only. Individuals may apply for assistance online at: www.myflorida.com/accessflorida/
You can (A) call your insurance provider,(B) call Palms Medical if you have our State/government/Medicaid/healthcare insurance for us who are on assistance ,(C) see if you have the guide they send out to you for your coverage...
Go on your My access account and upload. If not go yo the DCF office and scan it in.
Be thankful they gave them early. Everyone suffered some lost more then others. And most did not have food stamps to help them out.
