Do not go here unless you want to be charged three time for one visit. For about six months now I have been receiving bills saying that I owe them $20… Here’s the thing, I’ve paid it twice now and haven’t been since December. Their payment office are a bunch of scammers and people need to avoid this place like the plague.

Almost every month my doctor forgets to send in my refills and causes me to deal with withdrawal symptoms. Absolutely the worst agency in the state. This agency has caused the death of my aunt she had spots on her arms and the doctor said that they were age spots but she was only 62 yrs old all she had to do was a blood test and she would still be here she had leukemia and died because of the incompetence of Elizabeth demoski

ID, insurance, if no insurance then they would usually request proof of employment or pay stub. Any past medical records would be helpful too. If your previous physician is local they can also retrieve it from them via fax. Hope this is helpful.
