Called the Rolling Meadows office and the answering system did not indicate they were closed but upon arriving at the courthouse a police officer indicated that the office was closed until further notice. One would think that this office would have the mental capacity to inform people that they are closed when they call the listed number...

Go to the local county court house and request a copy, while you are at it get a few at least. In person should be available
I'm not sure the fee! But I recommend money order to proceed faster!
No they only require the parent of the the minor I'd only need the parents I'd only to obtain a birth certificate! I recommend you buy the first copy @15 and get a 2nd copy for only $4 for future reference!! My kids are grown and I was able to get there's with just my ID... Hopefully this info help!
